47: Supporting Hispanic Communities with a Person-Centered Model of Care

Our guest today is Sandra Rodriguez, the capacity development coordinator at Project Vida Health Center in El Paso, Texas. For over 20 years, Project Vida has been serving local communities of mostly Hispanic and Latino individuals, who according to the CDC are twice as likely to develop type 2 diabetes compared to their non-Hispanic white counterparts. Sandra will share some of the innovative ways her National Diabetes Prevention Program site engages with the local community and supports their health by addressing the body, mind and spirit of the individual.
